The consent form lets NSFAS check your and your family&#8217;s information; declaration forms confirm special circumstances, such as having no family details. Who signs depends on which form it is.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The NSFAS consent form<\/h2>\n\n\n\n<figure class=\"wp-block-table\"><table><thead><tr><th>Signature<\/th><th>Required when<\/th><\/tr><\/thead><tbody><tr><td>Applicant<\/td><td>Always<\/td><\/tr><tr><td>Father, mother or guardian<\/td><td>When your parents&#8217; or guardian&#8217;s income forms part of your household income<\/td><\/tr><tr><td>Spouse<\/td><td>When you are married<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Each person who signs fills in their name, ID number and the date. NSFAS&#8217;s 2026 consent form says:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>No electronic signatures are accepted.<\/li>\n\n\n\n<li>Complete it by hand, legibly, in black ink.<\/li>\n\n\n\n<li>Do not use correction fluid; any correction must be initialled by everyone who signs.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Declaration forms<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Declaration: Non-SASSA:<\/strong> for applicants who have indicated they have no family member details, including adults living independently of their parents.<\/li>\n\n\n\n<li><strong>Vulnerable Child Declaration:<\/strong> completed by a social worker for an applicant recognised as a vulnerable child by the Department of Social Development.<\/li>\n\n\n\n<li><strong>Disability Annexure A:<\/strong> for applicants living with a disability.<\/li>\n<\/ul>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\">Only sign for information you know is true.
